Shift: 8A-12P, up to 12hr shifts, hours may vary based on dept needs/unit census, includes days/evenings, rotating weekends/holidays and possible call

Summary:

To communicate an initial report of ED ambulance patients and gather pertinent data to initiate the care of the ED patient. Initiate emergency alerts based on guidelines.

Responsibilities:

Obtaining Patient Report

A. Demonstrates appropriate communication skills and knowledge to operate ambulance radio and collecting reports for each call. A report communication form will be completed on each patient to include: patient's current vital signs, current heart rhythm, basic demographic data, interventions, summary of patient complaint and estimated time of arrival. The report communication form is given to assigned nurse and/or physician. Notifies the charge nurse and physician of critical patients in route via ambulance.

B. Utilizes recorded line for all incoming report calls, including LERN.

C. Prioritizes patient triage by time sensitive patient complaints and initial report of patient. Communicates with Quick Look RN or designee for bed assignment or patient placement.

D. For time sensitive patient complaints - immediately contacts charge nurse or designee for patients in distress, shortness of breath/difficulty breathing, chest pain, altered level of consciousness, complaints of new onset numbness or new onset difficulty speaking, uncontrolled bleeding and severe pain.

E. Initiates stroke, trauma and STEMI alerts based on designated criteria.

F. Communicates with Quick Look RN and/or Charge Nurse for simultaneous ambulance arrivals and incoming LERN calls.

Outside Reports and Transmittals

A. Notifies physician of transmitted 12-lead EKG from field and ensures physician reviews all transmitted EKGs and places signature on the 12-lead EKG.

B. Responsible for handing off outside radiology reports/images to radiology for upload into the PACS system.

C. Completes form for tele-neurology notification and scans completed tele-neurology consultative report into electronic medical record (EMR).

Anonymous Registration

A. For patients who meet trauma alert criteria, enter the patient into the EMR utilizing the anonymous naming process.

B. For all other ambulance patients, completes bed hold process in the EMR.

C. Validates the patient appears on the Emergency Department's tracking board.

D. In collaboration with charge nurse, or designee, assigns care team, nurse and physician to incoming patient.

Other Duties

A. Contacts LERN (If LERN personnel does not call in trauma report) to ensure awareness of trauma patients.

B. Notifies Security of Air Med arrival.

C. Assist Unit Secretary with answering telephone calls and patient calls.

D. Obtain Vital Signs on patients

E. Assist with initiating IV/IO and/or drawing bloodwork on patients

F. Perform EKG

G. Obtain CBG

H. Assist in certain patient care activities such as transporting patient via wheelchair/stretcher and assisting in lifting/transferring of patients.

I. Insure adequate downtime documentation packets available for general ER patients as well as Stroke and Trauma downtime packets.

CUSTOMER SERVICE

A. Provides assistance, guidance, and directions to visitors, patients, employees,vendors and physicians.

B.Secures escort or directs patient to the appropriate service area.

C.Utilizes the 4 Cs in all interactions (refer to policy).

D.Maintains personal appearance following the department and facility's dress code.

COMMUNICATIONS

A.Establishes effective relationships with co-workers in own department as well as other departments.

B.Assist others and shows willingness to work as a team player.

C.Performs appropriately in difficult situations by remaining calm, notifying appropriate personnel and initiating appropriate action.

D.Responds to inquiries from patients, relatives, physicians and the general public quickly and efficiently by actively listening and taking appropriate action.

E.Discusses problems and/or confidential information only in appropriate setting and only with appropriate personnel. Keeps any confidential documents secure and out of the public view.

F.Calls for all service repairs as need is identified or enters repair ticket in electronic system.
