Description Duties and Responsibilities: Ensure the laboratory to be in conformance with current clinical laboratory standards, State of California, CLIA (Appendix G), College of American Pathologists, and OSHA. Laboratory Director Responsibilities For High-Complexity Laboratories The clinical laboratory shall be conducted, maintained and operated without injury to the public health and shall maintain records, equipment and facilities which are adequate and appropriate for the services rendered and demonstrate satisfactory performance in proficiency programs approved by Laboratory Field Services, Department of Health. Ensure that an appropriate test menu is performed that meets medical needs of our clients, that assays are optimal in generating reliable medical data, that turnaround times are medically useful to our clients and lead the laboratory in future testing by identification of medically important developments. Determine the scope and nature of procedures to control the reliability of test performance and personally monitor these control programs. Regularly assess the activities of the laboratory by personnel observation, evaluation and review of the reports of laboratory findings. Establish qualification criteria of laboratory personnel. Determine the format of laboratory report forms and decide what information is to be contained on these report forms. Regularly consult with supervisors and other staff members. Confer with those served by the laboratory on matters that relate to test performance and determine the nature and scope of technical and administrative information to be released by the laboratory staff. Be available daily in the laboratory and serve as Director of no more than three laboratories. Personally examine and report, or cause licensed, qualified supervisory technologists or licensed, qualified cytopathologists or pathologists to examine and report findings on those cytology specimens which have been defined by the Director to be abnormal or questionable. Personally examine and report, or cause licensed, qualified pathologists to examine and report findings on tissue specimens. Provide for, and assure that, the laboratory possesses adequate space including work surface to conduct and control the performance of all test procedures performed in the Laboratory. Provide for and assure that there is adequate area for safe storage and use of equipment and supplies. Provide and assure that all laboratory areas are well lighted and properly ventilated. Provide and assure that fume hoods and biological safety cabinets, properly installed and regulated, are used if required for safe performance of tests or for safe preparation of materials. Provide and ensure that instructions are available to be followed in case of fire and other emergencies and that such instructions are posted in a conspicuous place. Provide for and ensure that equipment, instruments, glassware and reagents are maintained in proper working order by periodic inspection, testing, or calibration in a manner acceptable to the Laboratory Field Services, Department of Health. Provide for and ensure that all reagents and stains shall be dated at the time of preparation and initialed by the person making the reagents or stains, or the date received and date opened if commercially prepared reagents or stains are used.  All reagents and stains shall be labeled to indicate identity, and titer, strength, or concentration.  Recommended storage temperature and expiration date, and other pertinent information necessary for quality control must be on the label. Provide for and ensure proper retention of records including maintenance for a period of at least two years of the following: Records of specimens received and tested, including identification of the patient, name of the submitter, dates of receipt and report, type of test performed, and test results. Records of inspection, validation, calibration, repair and replacement, to ensure proper maintenance and operation of equipment and proper reactivity of test materials. Records of quality control procedures in use in the various technical areas of the laboratory, including results on standards and reference materials and action limits when appropriate. Provide for procedures written in NCCLS format, and until the time such procedures are completed, card files, or flow charts for each procedure performed in the laboratory to include: Name of procedure Source of reference for the test method Date the procedure was last reviewed or modified by the                                     Director of Supervisor Current specific instructions for test performance Standards and controls required Instructions for collection and handling specimens to ensure test reliability Provide and ensure that the laboratory is conducting, maintaining and operating programs for controlling the quality of test performance in a manner acceptable to Laboratory Field Services, Department of Health, State of California. Provide and ensure that the laboratory shall have a sufficient number of qualified personnel to handle, under general supervision, the volume and diversity of tests performed to meet at least the standards of Laboratory Field Services, CLIA and CAP.
